Construction of World's Largest Telescope Begins in 2012 (SPACE.com)

The European Southern Observatory's plan to begin construction of the world's largest telescope ? the European Extremely Large Telescope ? will take a big step forward in 2012 now that early funding for the project has been secured.

The European Southern Observatory (ESO) governing body approved the 2012 budget, which paves the way for preparatory work to begin at the planned site of the European Extremely Large Telescope (E-ELT) at the Cerro Armazones mountain in the central part of Chile's Atacama Desert.

The telescope, which represents a joint collaboration between 15 countries, will be the world's largest ground-based optical telescope. Builders selected the observatory's location in Chile because of the optimal weather conditions in the area, where skies are clear overhead roughly 320 nights of the year, ESO officials have said.

The E-ELT's primary mirror will be a staggering 138 feet (42 meters) wide. For comparison, the Keck Observatory in Hawaii has a mirror that measures 33 feet (10 meters) wide, and the Subaru telescope, also in Hawaii, has a primary mirror that is 27 feet (8.2 m) wide.

The ESO governing council's approval of the 2012 budget includes funding for some initial work on the E-ELT project, such as the development of access roads to the telescope site and early construction efforts on one of the telescope's five mirrors. This work is expected to begin in early 2012, ESO officials said in a statement. [Stacking Up the 10 Biggest Telescopes on Earth ]

"The E-ELT is starting to become reality," ESO Director General Tim de Zeeuw said in a statement. "However, with a project of this size it is expected that approval of the extra expenditure will take time. Council at the same time recognizes that preparatory work must start now in order for the project to be ready for a full start of construction in 2012."

The ESO governing body is expected to make a final approval decision for the entire E-ELT project in mid-2012.

Plans for the E-ELT have seen steady progress over the past few months, ESO officials said. In October, ESO reached an agreement with the Chilean government that culminated in support for the project and the donation of land for the mega-observatory.

In that same month, an external review confirmed that the E-ELT could feasibly be built within the proposed budget of about $1.43 billion (1.082 billion Euros). Previous reviews also confirmed that the telescope's design is technically sound, ESO officials said.

Several ESO member states are already financially committed to the project, and funding is expected to be agreed upon between all members by mid-2012, which will enable the council to make its approval decision at that time, ESO officials said.

The E-ELT is expected to be operational early in the next decade, they added.

ESO already has three observatories in Chile, including the Paranal Observatory, which houses the Very Large Telescope, and the La Silla Observatory, which hosts the New Technology Telescope.

(AP) ? Crime scene investigators returned to the central Maine home from which a 20-month-old girl disappeared as police announced that they believe foul play was involved.

Two weeks after Ayla Reynolds went missing, evidence technicians from Massachusetts joined Maine State Police crime scene investigators on Friday at the Waterville home where the girl was last seen by her father. The home 75 miles north of Portland now sits empty, surrounded by yellow crime scene tape.

Hours later, Waterville police Chief Joseph Massey announced Friday night that the case "has evolved from the search for a missing child to a criminal investigation."

In a statement, the chief said the conclusion was based on evidence that has been gathered over the past two weeks, but he didn't elaborate. He said state police would take the lead on the investigation.

"All of our efforts continue to locate Ayla. Although this is beginning the third week, we remain hopeful," Maine Department of Public Safety spokesman Stephen McCausland said.

Ayla's father, Justin DiPietro, told police he last saw her when he put her to bed the night of Dec. 16. He reported her missing when she was nowhere to be found the following morning.

Before she vanished, Ayla was wearing green polka dot pajamas with the words "Daddy's Princess" on them and had a soft cast on her broken left arm. Extensive searches of woods, waterways, fields and private properties around Waterville, a city of 16,000 residents, have failed to turn up anything.

The day after Christmas, investigators announced a $30,000 reward, the largest ever for a missing person case in Maine, for information leading to Ayla's whereabouts.

McCausland on Friday declined to discuss whether any of the 300-plus leads had borne fruit for investigators. He also declined to talk about suspects or evidence that has been gathered. He said DiPietro and Ayla's mother, Trista Reynolds, of Portland, were cooperating with investigators.

There were news reports Friday that two cars seized from the Waterville home were returned to DiPietro and an unidentified woman. McCausland declined to confirm those reports.

Investigators put up crime scene tape at the father's home last week. Outside the home, teddy bears and stuffed animals were piled at a makeshift shrine.

Ayla was placed in her father's care while her mother was in a substance abuse rehabilitation program, which she completed.

Trista Reynolds, making an appeal on national television on Thursday, said that she had questions for DiPietro but that he had not returned her calls since their daughter went missing. She previously raised concerns about Ayla's treatment while in her father's care after the girl broke her arm, an incident described by police as an accident. She had no further comment Friday night, her sister said.

DiPietro couldn't be reached for comment. The Associated Press has been unable to find a telephone listing for him, and the house where he stayed is now empty.

A few days before Christmas, DiPietro, addressing the public for the first time, said in a statement he had "no idea what happened to Ayla or who is responsible." Later, in another statement, he said, "I would never do anything to hurt my child."

Former FBI profiler Clint Van Zandt said Friday that the odds of finding a child lessen if he or she isn't found within the first day or two of disappearing. But he said there's always reason for optimism, noting that there are even rare cases of missing children who turn up years later in someone's care.

"If you don't get this child back real quickly, you know that it gets harder and harder," he said. "But you can't give up hope."

Scott Bernstein, founder of Child Recovery International, a New York City-based organization that helps find missing children, agreed the first hours of an investigation are key in tracking down missing children as young as Ayla. Although the situation looks bleak, there's still room for hope, he said.

"One percent hope ? but I'll go for that 1 percent hope," he said.

After Ayla went missing, law enforcement officials likely divided their investigation into two parts, one team looking at people with access to her, such as relatives and family friends, and another group looking at the potential for an abduction by an outsider or stranger, Van Zandt said. Under both scenarios, he said, the odds are that the person who took Ayla knew something about her or her family.

Strangers' abductions of children do occur, but they're rare, accounting for only 105 to 115 children out of 750,000 to 900,000 missing-persons cases each year in the United States, Van Zandt said.

Van Zandt, who has worked similar cases, said Ayla's disappearance, which once had more than 80 searchers and law enforcement officers involved, has been difficult for law enforcers as well as for distraught family members.

"As an FBI agent working these cases, you never turn off the emotional porch light," he said. "You always leave on the light with the hope that the child will come home again."

Armenian, Greek clergy clash in Bethlehems Church of the Nativity

PanARMENIAN.Net - Dozens of priests and monks went at it with fists and broomsticks Wednesday, Dec 28, in Bethlehem's Church of the Nativity in the West Bank, police said.

The brief brouhaha broke out while about 100 Greek Orthodox and Armenian clergy were taking part in the annual Christmas cleanup at the 6th century holy site. Palestinian police were called in to restore order.

"No one was arrested because all those involved were men of God," Bethlehem police Lt. Col. Khaled al-Tamimi said, UPI reported.

The church, which is jointly administered by the Latin, Orthodox and Armenian churches, is spruced up every year between the Latin Christmas and early January, when the Orthodox sects celebrate Christmas.

Heat coach sees Coles, Chalmers as ideal partnership

-- The point, coach Erik Spoelstra said Wednesday, is that point guard should not be construed as an either/or proposition for the Miami Heat.

Amid the buzz created by the Tuesday breakout performance of rookie Norris Cole against the Boston Celtics, Spoelstra stressed what is most gratifying at this stage is the youthful versatility he has available at the position, when also factoring in fourth-year starter Mario Chalmers.

"We have two great, young point guards that we think bring two different things to the table that we really like," Spoelstra said before the Heat faced the Charlotte Bobcats at Time Warner Cable Arena, "and, most importantly, help our team win.

"We like it. They're both young. We feel as comfortable as anybody in the league, two young point guards under 25 that bring different skill sets."

To a degree, having dual young point guards almost is emerging as an NBA trend. Wednesday, it had the Heat going against the Bobcats' D.J. Augustin and Kemba Walker. Friday, at the back end of this two-game trip, it will have the Heat going against the Minnesota Timberwolves' Ricky Rubio and J.J. Barea.

"The fact that both of them are young and promising with a lot of potential," Spoelstra said of Chalmers and Cole, "we haven't had that in this organization for a long time."

Spoelstra remains committed to Chalmers as starter, if only because of the game-changing impact Cole can produce off the bench.

"With Norris, immediately you see it. The pace changes," Spoelstra said. "He ignites it, the break. He knows how to play fast. He gets people involved on the break, and we like that."

While much has been made about the Heat's upgraded offense, including some who question whether it could succeed in the playoffs, Spoelstra said the detractors are missing the point about what remains the team's core identity.

"This team is not built for and we won't accept trying to outscore teams," he said. "That's not how we feel we'll have any success in the playoffs."

Instead, Spoelstra said the priorities remain defense and rebounding.

"So we have to stay committed," he said, "and particularly if we're going to see different schemes, gimmick defenses, that we can still control it by defending and rebounding the ball and getting out in the open court.

"And it starts on that end. Our guys understand what makes this successful, and it's not trying to outscore other teams."

At one point during Tuesday's victory over the Celtics, Boston coach Doc Rivers shouted to his players that the Heat only could score at the rim. Spoelstra has no issues there.

"That is our identity," he said. "That's how we're built. So a lot of things that you're seeing us do, that we built on from last year, are to take advantage of those things, get out in the open court, open up driving lanes, take advantage of these guys in the post and play out of our offense, a little bit more movement, a little bit more pace."

Even with the Heat struggling Tuesday against the Celtics' zone, Spoelstra elected to keep forward James Jones on the bench, despite Jones standing 6 of 8 on 3-pointers this season to that point.

"When we face zone, we still have to stay true to our mentality," Spoelstra said. "And sometimes that means we'll have 3-point shooters and we have to take advantage of that, keep defenses honest. But we also have to be able to execute to get opportunities in the paint."

(AP) ? Soccer-playing elephants used all four feet and even their trunks trying to score goals. Racing pachyderm thundered to the finish line to the cheers of the crowds. And in the elephant beauty pageant, contestants sported nail polish on their not-so-dainty toes.

It was all part of an elephant-themed festival in Nepal that wrapped up Wednesday. The three-day event was held to promote conservation awareness and lure foreign visitors to Nepal.

The elephants were trained for weeks for the games, taking time off their normal jobs carrying tourists through protected jungles near Chitwan. The conservation forest has rhinos, several species of deer and crocodiles and is a popular tourist spot some 106 miles (170 kilometers) south of the capital, Katmandu.

"We hope that the elephant festival will help bring more tourists to Chitwan. We need both foreign and domestic visitors," said Ghanashyam Shrestha, one of the organizers.

Tourism is picking up in Nepal as it slowly recovers from a 10-year Maoist insurgency that killed more than 13,000 people. The conflict ended after the rebels gave up their armed revolt and joined a peace process in 2006.

But the tourists who mainly come to hike the Himalayan country's many mountains aren't returning fast enough for some. Nepal received some 600,000 visitors in 2010, short of the goal of 1 million set by the government declaring the Nepal Tourism Year.

Organizers of events like the elephant festival ? which draws on a popular elephant polo event held elsewhere in Nepal ? hope more colorful events will increase interest in tourism.

The final event, a 300-meter race, was won by an elephant named Bajadur Gaj, who pounded his way to the finish line in 69 seconds as thousands of locals and foreign tourists cheered.

Teams of four elephants also played soccer matches using a standard-size ball . The elephants blocked passes, kicked the ball and batted it with their trunks, pushing each other for control of the play.

"Training the elephants to play soccer was not easy but they learned the basic command. They understood they need to hit the ball when I yell 'kick' at them," said Basudev Mahato, 37, an elephant mahout who has been training and riding elephants for 15 years.

The elephants are between 4 and 5 years old. Young ones are easier to train and run faster, Mahato said.

At the Hattishar elephant camp, trainers and workers cleaned up the elephants, fed them a special meal of rice and sugar wrapped in grass and painted them to prepare them for the event.

Over at the beauty pageant, a trainer painted white patterns on an elephant named Loktantrakali, then varnished her toenails bright red.

Judges ? who checked contestants bodies' for scars and overall beauty and also watched them perform tricks ? picked Loktantrakali as the second-prize winner.

Japan hopes Europe will boost rescue mechanism: sources

TOKYO (Reuters) - Europe should boost the total firepower of its rescue fund and frontload its funding to send a positive signal to investors and international partners that it is determined to solve its debt crisis, Japanese officials said on Monday.

Japan has repeatedly expressed its willingness to help Europe contain its debt crisis, but has also stressed it wanted to see a convincing action plan before making any firm commitments.

"Japan like other non-euro countries is prepared to do something, but unless European countries take decisive action it is hard to make those steps effective," a senior Japanese government official said.

Lifting the combined size of the current bailout fund (EFSF) and the new permanent European Stability Mechanism (ESM) beyond the current 500 billion euros would be a major step and an encouraging signal.

"We expect European countries will review the combined ceiling of 500 billion euro of EFSF (European Financial Stability Fund) and ESM in a very positive manner," the official told Reuters.

European leaders agreed in Brussels earlier this month to accelerate the launch of the ESM by a year to mid-2012 with an effective lending capacity of 500 billion euros ($650 billion), but questions have arisen about the size and timing of contributions.

Japanese officials said that while bringing forward the launch of the fund was positive, a more ambitious ceiling might be needed given that Europe had little success in bringing in outside investors to boost the firepower of the EFSF fund.

"The leveraging of EFSF money by investors' money doesn't look like materializing very well. That's why they are frontloading the ESM and the review of the ceiling of 500 billion euro is very important," said the official, who declined to be named.

"European countries may think what they've already decided is a major step forward, but markets want Europe to act more decisively."

German Finance Minister Wolfgang Schaeuble signaled over the weekend that Europe's biggest economy and its main paymaster could boost its contribution to the fund and support its swift launch, although any decisions would have to be made in January.

Since the beginning of the crisis more than two years ago, European leaders have orchestrated bailouts of Greece, Ireland and Portugal, set up a euro zone rescue fund and earlier this month agreed to boost the International Monetary Fund's resources by 150 billion euros.

Still, throughout the crisis that has also shaken Italy and Spain, investors have repeatedly been left with the impression that whatever was agreed in Brussels was too little, too late.

Japan, the United States, Canada and others have voiced their frustration with Europe's piecemeal progress and repeatedly called for bold steps that would create effective "firewalls" around the euro zone's weaker, heavily indebted economies.

Another Japanese government official reiterated on Monday that Tokyo, which led an international effort to boost the IMF's coffers after the Lehman crisis, was open to contributing more but that its decision depended on Europe's actions.

Officials in Tokyo said markets needed to see both effective defenses in the form of funds sufficient enough to cover the crisis-hit nations' financing needs and commitments to fiscal discipline.

"Fiscal discipline is very important. Even if we provide firewalls we need fiscal discipline," the official said.

While Tokyo has repeatedly voiced concern about developments in Europe, its plans to buy Chinese government debt did not reflect lack of confidence in the euro or U.S. dollar assets, another official said.

He said the plans, discussed during Prime Minister Yoshihiko Noda's visit to Beijing, aimed at strengthening economic ties between the two nations rather than diversifying Japan's exchange reserves, mostly made up of dollar and euro assets.

"The idea is not to depart from the dollar or U.S. government bonds or the euro, so it should not be interpreted as diversification of our portfolio," the official said.

"I don't have any doubts about creditworthiness of the dollar or U.S. government bonds. The dollar will remain the most important currency for the foreseeable future."

GOP: Gingrich, Perry fail to qualify for Va. primary

Former House Speaker Newt Gingrich and Texas Gov. Rick Perry have failed to qualify for Virginia's March 6 Republican primary.

The Republican Party of Virginia announced late Friday and early Saturday that Gingrich and Perry fell short of the 10,000 signatures of registered voters required for a candidate's name to be on the ballot.

Former Massachusetts Gov. Mitt Romney and Texas Rep. Ron Paul met the threshold and will be on the ballot.

Failure to compete in Virginia, which is among the "Super Tuesday" primaries, would deal a huge blow to any contender who had not locked up the nomination by then.

State GOP spokesman Garren Shipley said volunteers spent Friday validating petitions that the four candidates submitted by the Thursday 5 p.m. deadline to the State Board of Elections.

"After verification, RPV has determined that Newt Gingrich did not submit required 10k signatures and has not qualified for the VA primary," the Republican Party of Virginia announced early Saturday via Twitter.

Shipley was unavailable for comment on the announcement.

The 10,000 registered voters had to include 400 signatures from each of Virginia's 11 congressional districts.

The development is a major setback for Gingrich, who has tried to use his recent upsurge in popularity to make up for a late organizing start.

A Quinnipiac poll of Virginia Republicans released earlier in the week suggested that Gingrich had a slight lead over Romney.

Gingrich said Wednesday he had enough ballot signatures, but he wanted to come to Virginia to deliver them personally. Taking no chances, his volunteers asked everyone to sign petitions before entering Gingrich's rally Wednesday night in Arlington, just across the Potomac River from Washington.

Gingrich represented Georgia in Congress for two decades, but has lived in McLean, Va., an upscale Washington suburb, while he's worked in the private sector.

Gingrich's early-December rise in several polls gave him renewed hopes of carrying his campaign deep into the primary season.

In a statement, Perry's campaign described the signature shortfall in Virginia as a "isolated situation."

"We will closely review the facts and law to determine whether an appeal or challenge is warranted," the statement added. "Governor Perry has the utmost respect for the strong place Virginia holds in our nation's economic and military strength and in American history. He will continue to work hard to build strong support in Virginia and earn the trust of conservative-minded voters there."

It was unclear if Minnesota Rep. Michele Bachmann, former Pennsylvania Sen. Rick Santorum or former Utah Gov. Jon Huntsman submitted petitions to the state board.

Meanwhile, Virginia's Democrats said President Barack Obama's re-election campaign gathered enough signatures to get him on the state's primary ballot.

As he is the only candidate who qualified, the state's Board of Elections will likely cancel the scheduled primary election.

Game Informer is back to its teasing ways. Last month, the magazine?s staffers somewhat hyped up last month?s issue by saying the January cover ?may be the most surprising of the year?. This turned out to be South Park: The Game.

This time around the folks over at Game Informer believe that the developer behind their upcoming reveal will be shocking. When asked if fans could potentially guess the mystery game, editor-in-chief Andy McNamara said on Twitter:

?I think you could (though it?s not announced), but doubtful you would get the real nuts which is the developer.?

He added, ?I can pretty much promise not many will see this one coming?.

We?re quite a ways away from the announcement.?McNamara confirmed that they?ll be showing Game Informer?s next cover on January 5.

Hollywood dialect coach Robert Easton dies at 81 (AP)

LOS ANGELES ? Character actor and Hollywood dialect coach Robert Easton, whose successes include teaching Forest Whitaker to speak like Idi Amin in the 2006 movie "The Last King of Scotland," has died in Los Angeles. He was 81.

Daughter Heather Woodruff Perry tells the Los Angeles Times ( http://lat.ms/rEfhAQ) that Easton died of natural causes on Monday at his home in the San Fernando Valley.

His movie credits include "Paint Your Wagon," "Pete's Dragon," "Pet Sematary II" and "Primary Colors."

When he was younger, he mainly played country bumpkins on TV shows because of his Southern drawl.

He feared being typecast so he worked on different accents and learned he could mimic regional speech patterns.

As a dialect coach, he worked Arnold Schwarzenegger, Charlton Heston, Liam Neeson, Anne Hathaway and Robert Duvall.

Health panel takes heat on cancer screening advice (Reuters)

WASHINGTON (Reuters) ? Dr. Ned Calonge knows firsthand how hard it is to tell Americans they'd be better off with fewer routine medical tests.

A long-time family doctor in Colorado, Calonge presided over the U.S. Preventive Services Task Force, an influential government-backed panel of health experts, when it said that most women under 50 could skip their regular mammograms.

The recommendation two years ago challenged the conviction of many breast cancer patients that they survived precisely because they were screened early. It unleashed a public fury that has weighed on the panel's deliberations ever since.

"We blew the message," said Calonge, now president and CEO of the Colorado Trust foundation. "The nuance was completely gone."

Two men phoned in death threats to Calonge. Protesters showed up by the offices of the government agency that supports the panel, tucked away in a Maryland suburb. The furor slowed down work on a decision to limit prostate cancer screenings as President Barack Obama fought to pass his signature healthcare law and his Democratic party faced a mid-term election challenge in 2010.

"There was a lot of pressure from above to be more careful politically and orchestrate things better," said Dr. Kenneth Lin, who at the time was an officer at the Agency for Healthcare Research and Quality (AHRQ), a Department of Health and Human Services entity that supports the panel. "Everything with the word 'cancer' got shoved back."

Calonge rotated off the panel this past March after eight years, while Lin quit AHRQ late last year in protest over the delay to prostate cancer screening guidelines that were only released in October. A White House official noted that Calonge has attributed the delay in a final decision on prostate cancer screenings to scheduling conflicts.

Their experience shows just how difficult it will be to curb spiraling costs in the world's most expensive healthcare system by determining what screenings work, based on a rigorous study of clinical evidence, and what can lead to unnecessary and risky procedures.

"More screening is not always better," said Dr. Christine Laine, a general internist and editor of the Annals of Internal Medicine who is not part of the panel. "That message is lost in healthcare in general."

The U.S. Preventive Services Task Force is right on the firing line. For much of its 27-year history, it helped convince millions of Americans to get screened early for disease.

Now the panel of primary care doctors, nurses and academics has reviewed a growing body of research that shows some early screening harms more people than it helps. But it has struggled to convince patients and doctors.

In the wake of the mammogram guidelines, the rate of such screenings for women aged 40 to 69 was barely changed in 2010 compared with 2009, according to the National Committee for Quality Assurance.

"We have a public health measure that we know is effective. Why is it continually being questioned?" said Dr. Carol Lee, breast imaging commission chairwoman at the American College of Radiology.

BROACHING THE NEGATIVES

The public at large is no less skeptical. A recent Gallup poll showed that nearly 60 percent of Americans believed that standard cancer screenings - including mammograms and prostate specific antigen (PSA) blood tests - were performed often enough. Thirty-one percent thought they should be conducted more frequently. Only 7 percent said they were done too often.

"It's extraordinarily hard to give up the notion that there's a way to protect yourself from dying from cancer... Our goal here is to make it a matter of evidence, not a matter of opinion," said Virginia Moyer, a pediatrician from Baylor College of Medicine, who now chairs the 16-member panel.

"Our successes are measured in positives," she said of the public's growing awareness of screening in the last three decades. "We are just beginning to approach the negatives."

Burned by the experience with mammograms, the task force is looking for a better way to deliver the message, consulting with powerful consumer interest groups, hiring public relations professionals and reworking some of the language tied to its system of letter-based recommendations.

"We're spending more time paying attention to how we say things to make sure it's understood well," said long-time panel member and current co-vice chair Dr. Michael LeFevre, a professor of family medicine at the University of Missouri School of Medicine. "We have no interest in being some wizard behind the curtain."

The panel now issues its recommendations in draft form first and solicits public comment before making them final. In about a year, the public may have a chance to chime in early on the evaluation process, including posing questions for researchers and reviewing the evidence report draft used by the panel.

Task force officials concede that the comments are unlikely to change the recommended letter grade, unless they introduce crucial new evidence. But they can point to misunderstandings and help the panel better craft its message.

In late October, the panel met with consumer interest groups, including retired persons lobby AARP and the Consumers Union, to get input on how to frame recommendations that was once reserved for patient advocates.

The public's participation has been unprecedented. The panel is now finalizing its PSA prostate cancer recommendation and public comments on the subject have reached into the thousands, LeFevre said.

WEIGHING THE EVIDENCE

The 2009 mammogram guidance from the task force was based on the panel's assessment of new research that showed most women over 40 face a 3 percent risk of dying from breast cancer if they have not been screened. Beginning mammogram screening at age 50 and following up every other year reduced that risk to 2.3 percent, compared with 2.2 percent risk starting at age 40.

An extra decade of screening could invite harms such as unnecessary biopsies and tests, the possible treatment of non-deadly cancers and radiation. Women in their forties are also more likely to receive false positive results.

Another view of the data showed that starting screening at age 40 led to 5,000 more mammograms, 500 false positive results and 33 biopsies for every breast cancer death prevented, according to LeFevre.

"If it was just how many deaths do you cause versus how many deaths you prevent, that would be too easy, that would be simple math," LeFevre said. "We start with somebody who feels well, and we risk making them feel worse."

The panel voted on a "C" recommendation, which calls for patients to decide on the screening with their doctor. But when the recommendation came out in November 2009, it started with a sentence saying the panel "recommends against" routine mammograms for most women under 50, and that language triggered the controversy.

Under pressure, the task force dropped the phrase "recommends against" a month later. Its rating on mammograms remains a "C."

The American Cancer Society questioned the evidence, saying the panel focused on gold-standard clinical trials but weeded out newer observational studies that showed better results.

"Screening is not perfect and it's not error-free, but the question is... do you take protective measures against the unlikely probability that you develop cancer... or do you take your chances?" said Robert Smith, director of cancer screening at the ACS.

That calculation still appears to be guiding doctors, either out of concern of missing an early sign of disease or fear of lawsuits, health experts said.

"Shared decision-making (between doctors and patients) sounds nice, but in practice usually you just end up doing the test," said Dr. Roger Chou, an internist and researcher at the Oregon Evidence-Based Practice Center. Chou authored the report on prostate cancer behind this year's task force recommendation.

POLITICAL RUMBLINGS

The heat over mammograms weighed on deliberations over prostate cancer screening. In 2008, the task force gave an "I" recommendation on the PSA test in healthy men under 75, which meant it had insufficient evidence to make a call.

The panel usually updates its recommendations every five years, but new research published in 2009 warranted an earlier evaluation. One U.S. study showed a slightly higher risk of death for men with no symptoms of illness who received a PSA test, while European research showed a slightly lower risk of death.

Although the PSA blood test itself is innocuous, data reviewed by the task force also showed that 90 percent of American men who tested positive got treated, even if they may have been able to forego it, LeFevre said. Out of 1,000 men treated, five would die, 70 would have serious complications and 200 to 300 would be impotent or incontinent.

Given the possibility of false positives in the screening and the fact that prostate cancer can take many years to progress and show symptoms, the question is whether those risks are greater than the risk of doing nothing.

"It looks like your chance of being alive and well is greater if you don't get screened than if you do get screened," LeFevre said.

In November 2009, task force members voted on a stronger "D" rating on PSA tests, meaning they recommended against the prostate cancer screening in men under 75.

But the timing was poor as Obama struggled to win over a majority of lawmakers for his healthcare overhaul and Congressional elections loomed large. Once the law was passed in March 2010, it brought more attention to the task force by mandating insurance coverage of services it does recommend.

Republicans opposed to the bill used the mammogram example to show how government could intrude on life or death decisions. The task force's "C" and "D" recommendations don't dictate insurance coverage, but Congress quickly turned around legislation to make sure insurers covered mammograms for women in their forties.

"The thought that my work was being use as a fulcrum by one party to kill the most substantial part of healthcare legislation since I've been in practice? I've got to tell you, that's something to lose sleep over," Calonge said.

Officials working with the panel heard that more controversy could threaten the task force budget, up for Congressional approval. In 2010, Health Department funding for the panel was $4.3 million. This year, the agency overseeing the panel spent about $11 million on work related to the task force.

Calonge says the panel wanted more evidence of how the tests could harm healthy patients, and ordered further research. He canceled a new vote on PSA screenings in November 2010, citing scheduling problems, a decision that was widely criticized.

"In my heart of hearts I'd really like to believe that we'd delay it anyway," without the surrounding politics, Calonge said. "We were trying to make the recommendations solid."

That was too much for Lin, who believed the evidence was already enough to show the public was at risk. After talking with his pastor and his wife, he quit AHRQ.

"Even delaying it for a few months, much less a year, it was really relegating the men to the harms they were exposed to," Lin said.
